Organization Change Request Form

This request applies to College Board enterprise level organization data.
Requests are expected to be processed in the order received and within plus three business days.
Please verify organization details before submitting requests. Data dependencies or need for clarification may delay processing.

For organizations with School (AI) Codes: Updates to name, address, status, URL, phone, email, and grade levels must be submitted through the School Code Request Form, https://schoolcode.collegeboard.org/, by web-verifiable employee of the school or district. Requests for new codes should be submitted by head of school/district or authorized designee through the School Code Request Form.

For organizations with Score Send (DI) Codes: Requests for new Score Send Codes or updates to name, address, status, URL, phone, email, and SAT or AP program contact information must be submitted through the Score Send Code Request Form, https://scoresendcode.collegeboard.org/. Requests for new codes must be submitted by Registrar's Office, Director of Admissions, or Dean of Students. Requests to update existing codes can be submitted by a web-verifiable staff member.

Contact information of the individual filling out this form, i.e., of the College Board staff or customer service member if conveying a customer request. Note the completion status will be emailed to the Requestor Email Address only.
